How To Become A Client
It’s easy to become a client of Rochedale-Springwood Meals on Wheels. Just follow these simple steps:
Step 1:
Complete our online client application form by using the green button above.
If you need any assistance, phone our friendly staff on (07) 3841 6367 between 8:00am and 12:00pm.
(If calling outside normal business hours please leave a message on the answering machine)
Step 2:
Once we receive your information, one of our office staff will contact you to arrange a time for an interview. You or your carer will need to complete some other paperwork including a My Age Care Form.
Step 3:
That’s it .... we can start delivering meals the very next day.
You can contact us yourself to become a client or you can have someone else contact us. We accept referrals from family, friends, neighbours as well as medical professionals. A referral from your doctor is not necessary.
